After IV thrombolysis, obtain follow-up CT/MRI at 24 hours and exclude haemorrhage before starting antiplatelet or anticoagulant therapy. Earlier treatment is reserved for exceptional compelling concomitant indications under stroke-team guidance. After thrombectomy, determine timing according to associated IV thrombolysis, stenting, procedural factors and bleeding risk. Source: AHA/ASA 2026 guideline.
